A 68-year-old woman who has had 2 acute coronary syndrome episodes in the past 5 years was placed on a regimen of rivaroxaban 2.5 mg twice daily plus once daily low-dose aspirin. At her yearly evaluation she reports feeling generally well but did mention feeling fatigued and sometimes notices a fluttering sensation in her chest. She was fitted with a 24-hour Holter monitor which revealed atrial fibrillation.
