Target Audience and Goal Statement

This activity is intended for primary care providers (PCPs), diabetologists/endocrinologists, and nephrologists.

The goal of this activity is to enhance learners’ knowledge and competence about the early identification and appropriate management of patients with type 2 diabetes (T2D) and chronic kidney disease (CKD).

Upon completion of this activity, participants will:

  • Have increased knowledge regarding the
    • Prompt identification of patients with diabetic kidney disease (DKD)
    • Recent findings of cardiovascular and renal outcome trials with modern diabetes drugs
  • Have greater competence related to
    • Proactively screening renal function regularly, in all patients with T2D
