Target Audience and Goal Statement

This activity is intended for cardiologists, primary care physicians, nurses, and all clinicians interested in caring for patients with heart failure.

The goal of this activity is to improve patient self-management through better communication designed to engage and bring patients into their plan of care.

Upon completion of this activity, participants will be able to:

  1. Recognize effective patient self-management tools and strategies in the setting of heart failure (HF)
  2. Identify patient-directed strategies to improve adherence to therapy for patients with HF
  3. Assess healthcare provider-patient communication strategies to improve adherence to therapy for patients with HF
