Target Audience and Goal Statement

This activity is intended for cardiologists, emergency department physicians, critical care specialists, nurses, and all clinicians interested in the care of patients living with heart failure (HF).

The goal of this activity is to enlighten clinicians on various methods used to achieve optimal medical therapy for patients with chronic HF.

Upon completion of this activity, participants will be able to:

  1. Assess the utility of biomarkers in guiding therapy for chronic HF
  2. Examine the role of patient-centered medical homes (PCMHs) in care coordination for patients with chronic HF
  3. Identify strategies to optimize medical therapy for patients with chronic HF
