Implantable cardioverter-defibrillators (ICDs) prolong life in patients with depressed ejection fractions (EFs), especially in cases where the patient has had a cardiac arrest or a life-threatening arrhythmia. However, there is a larger group of patients who are potentially at risk for SCA, which includes those who have coronary disease, may have been recently revascularized, or have a reduced EF but have not yet had an event. A team approach with the interventionalist and electrophysiologist along with the nursing team needs to consider all the options during this vulnerable period following percutaneous coronary intervention (PCI). ICDs are not indicated within the first 40 days, yet this is the highest-risk period for sudden cardiac death. Drs. Ted Feldman and Westby Fisher are joined by nurse practitioner Rena Silver for a discussion on options for getting the post-myocardial infarction/post-PCI patient through this difficult period safely.
